Overview Join our dynamic healthcare team as a Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse clinical settings. Responsibilities Conduct thorough physical examinations and patient assessments, including vital signs, medical history reviews, and physiological evaluations. Develop and implement individualized treatment plans encompassing medication administration, injections, IV infusions, and other procedural interventions. Manage acute and chronic conditions across various specialties including primary care, geriatrics, pediatrics, behavioral health, infectious disease care, and senior care. Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Cerner to document medical records accurately while ensuring compliance with HIPAA regulations. Perform diagnostic evaluations using laboratory testing, sonography, medical imaging, or specimen collection/processing techniques; interpret results to guide clinical decisions. Provide patient education and health coaching tailored to individual needs; support discharge planning and case management for complex cases such as hospice & palliative medicine or post-acute care. Engage in telehealth services to extend access to care remotely while maintaining high standards of patient assessment and triage. Qualifications Valid Family Nurse Practitioner certification with current state licensure; experience working with children, seniors, or individuals with disabilities is highly desirable. Demonstrated proficiency in EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks; familiarity with CPT coding, ICD-9/10 coding is preferred. Critical care experience including ICU or Level I/II trauma center exposure; hospital medicine or emergency room experience is advantageous. Skilled in performing procedures, and sterile techniques. Knowledge of pharmacology related to psychopharmacology, pain management, infection control protocols and medication administration best practices. Experience in diverse clinical environments including outpatient clinics, inpatient units, nursing homes or assisted living facilities; home care experience is a plus. Strong understanding of medical terminology and physiology; ability to perform comprehensive physical examinations and diagnostic evaluations confidently. Excellent communication skills for effective patient education and collaboration within multidisciplinary teams; commitment to maintaining HIPAA standards at all times.
