Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. This position offers an exciting opportunity to work in a fast-paced environment where your proactive approach and clinical proficiency make a meaningful difference every day.DutiesRelay on our multi-disciplinary team to complete initial biopsychosocial examinations and patient assessments to identify health issues accuratelyDevelop and implement individualized treatment plans for a wide range of acute and chronic conditionsUtilize EMR (Electronic Medical Records) system Practice Fusion (PF) to document patient encounters comprehensivelyProvide health coaching, education on disease prevention, and guidance on lifestyle modifications to promote wellnessRequirementsValid Family Nurse Practitioner certification/license with current state registrationProven experience in primary care settings with proficiency in outpatient clinic operationsStrong knowledge of addiction medicine and mental health care.
