Position Summary The Family Specialist serves as a key member of the ACT Team, providing leadership in integrating family-focused goals into treatment planning and service delivery. This role engages families and support systems to promote recovery through psychoeducation, coordination of care, and evidence-based interventions. The Family Specialist delivers services both in-office and in the community, supporting individuals in achieving social and familial stability. Essential Responsibilities Assess family dynamics and social supports to develop appropriate interventions that promote overall wellness. Lead discussions and interventions related to family engagement, incorporating findings into individualized treatment plans. Engage families, with proper consent, to provide education on mental health conditions, treatment, and available community resources. Develop and coordinate person-centered treatment plans in collaboration with the multidisciplinary ACT Team. Coordinate case management services to ensure comprehensive and integrated care delivery. Collaborate with team specialists (e.g., substance use, employment, nursing) to support holistic service provision. Serve as a liaison between the ACT Team and external providers, agencies, and community resources. Conduct field-based visits, with the majority of services delivered in the community. Provide crisis intervention and mental health support as needed, including participation in on-call rotation. Document all client interactions, progress notes, and required data in the EHR system within established timeframes. Maintain compliance with agency policies and regulatory requirements, including OMH and AOT standards. Participate in team meetings, case conferences, and clinical consultations to support quality care. Assist individuals in developing independent living skills, including budgeting, self-care, nutrition, transportation, and community integration. Support individuals in accessing and maintaining community resources, including accompanying them to appointments as needed. Enter and maintain accurate service data in designated systems (e.g., CAIRS, Vellohealth). Communicate relevant clinical and operational updates to leadership and team members in a timely manner. Participate in required trainings, including OMH-approved ACT trainings and ongoing professional development. Maintain confidentiality and adhere to ethical and professional standards at all times. Perform additional duties as assigned to support program operations. Additional Requirements Ability to work a flexible schedule, including evenings, weekends, and on-call coverage. Ability to travel and conduct community-based work; valid driver’s license preferred. Ability to operate an agency vehicle, if required. Supportive Environment: A collaborative team that values your contributions Qualifications One year of training and/or experience in family assessment and intervention, psycho-education and/or other family support services, including cognitive-behavioral strategies. Must hold a master’s degree in social work or related human service field. Experience working with behavioral health population preferred. Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record. Valid New York State driver’s license and vehicle required – candidates are subject to regulatory background checks and must be willing to utilize own vehicle for home visits.
