Job Summary: Work with the Chronic Diseases Prevention and Self-Management Program team to enroll or lead program classes. Job Duties: Program Recruitment (50%): Recruit program participants, adhering to each program's eligibility criteria. Participate in recruitment events, pitching the program information to potential participants. Participate in investigator meetings and workshops. Adheres to internal controls and reporting structure. Lifestyle coach and leader (50%): Conduct Chronic Diseases Prevention and Self-Management Program classes, as assigned. Collect and report point-of-care blood work, biometrics, and surveys from program participants. Record and report participant data and outcomes. MARGINAL OR PERIODIC JOB FUNCTIONS: Performs related duties as required.
