The Field Based Community Health Worker is responsible for identifying and initiating referrals for social service programs, including financial, psychosocial, community and state supportive services. As a Field Based Community Health Worker (CHW), you will act with Medicaid members to ensure to provide home and social resources and member resource education. The Field Based Community Health Worker also addresses social determinants of health such as transportation, housing, and food access. Working Schedule: This position is full time, 40-hour work week, Monday - Friday between the hours of 8:30am - 7:00 pm Central Standard time.
