Our Own Lives is hiring an Adult Services Program Assistant to do exactly that! Pay is $18.27/hour. Previously the direct services arm of Community Connections, we have decades of experience providing person-centered, inclusion-focused supports to the disabled community on its own terms. Title: Program Assistant for Adult Services Reports to: Adult Services Program Manager FLSA: Hourly/Non-exempt Full-time Job Description: This position assists program managers with the administration of one or more of the organizational programs. It is a nonsupervisory position. Responsibilities Advance the mission, vision and values of Our Own Lives while performing the responsibilities of this position. Ensure accurate and up to date information in the THERAP system. Provide regular back-up as a Direct Support Professional by working shifts in direct care. Help coordinate quarterly home visits and other monitoring tasks, as well as conducts monitoring and visits, and documents findings in Therap. Monitor client goals for accuracy and implementation. Completed in 3,6,9-month intervals. After manager review, enter consultation forms, informed consents, protocols and assures information is in THERAP. Monitor client information in the Therap system and ensure the information is accurate. Coordinate program’s preparation for Service Plan meetings to ensure Person-Centered Thinking. Enter ISPs into Therap per the Service Plan meetings and maintain current information as plans change. Assist program managers in obtaining information they need to complete their responsibilities. Attend service plan and IDT meetings as needed. Assist with identifying and locating new opportunities in the community for day program services. Assist with monitoring and generating documentation and attendance for monthly billing. Assist in the preparation of cover sheets and other information for Human Rights Committee sessions. Ensure all information needed is on SharePoint by designated due date. Attend training and meetings as needed. Perform critical incident investigations as needed, if trained. · Required to be on call days, nights, and weekends as part of the emergency phone rotation with other Our Own Lives personnel · Support management staff with other projects, compliance, and documentation requirements as needed. · All other reasonable duties as assigned. If driving, they must possess a drivers’ license in the state they reside and current auto insurance * Satisfactory completion of all background checks and references, including Motor Vehicle Record is required Minimum Qualifications: High school graduate/College graduate preferred. Previous management skills helpful. Lived experience will be considered in the leu of higher education and heavily valued. All training requirements for Our Own Lives must be completed. Must be able to participate in rotations for on call phones this includes nights and weekends.
