We are seeking a compassionate and dedicated Nurse Practitioner (NP) specialized in Palliative Medicine to join our healthcare team in Allen County, OH, near Cleveland. The successful candidate will provide comprehensive palliative care to patients with serious, chronic, or life-limiting illnesses, focusing on symptom management, quality of life, and patient-centered care. Responsibilities include conducting detailed assessments, developing individualized care plans, collaborating with interdisciplinary teams, and supporting patients and families through complex medical decisions. The NP will also educate patients on disease progression and treatment options, ensuring compassionate communication and emotional support. Key skills required are excellent clinical judgment, strong communication skills, cultural sensitivity, and the ability to work independently and as part of a team. Knowledge of palliative care principles and experience with end-of-life care are preferred.
