Work Experience Under general supervision, serves as a leader, change agent and coordinator of patient care services for GI Oncology patients. Takes a leadership role in overall design and implementation of a patient centered care delivery system that facilitates achievement of patient outcomes, supports continuity of care and enhances patient and care giver satisfaction. Accountable for the processes and outcomes of care during an episode of care. Accountable to peers for the advancement of group practice and to the supervisor for the totality of work performance. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified. Serves as a role model and clinical resource for expert care and outcomes management. Assists staff in developing and improving skills in managing therapeutic relationships with patients and families. Assesses staff performance in documentation and provides feedback. Monitors improvement plans. Develops and implements systems to improve documentation. Assesses and improves preoperative patient instructions. Responsible for patient and family education. Monitors processes and outcomes of patient education; implemen
