CareCore at Mary Scott is seeking an experienced and motivated Healthcare Outreach Liaison to help grow community partnerships and drive referral development for our skilled nursing and rehabilitation facility in Dayton, Ohio. This role is ideal for someone who enjoys relationship building, networking, and being out in the community while making a meaningful impact in post-acute care. The Healthcare Outreach Liaison will work closely with facility leadership and report directly to the Regional Director of Business Development. Responsibilities: Build and maintain strong referral relationships with hospitals, assisted living facilities, physicians, case managers, social workers, senior centers, and community organizations Conduct regular outreach visits throughout the Dayton market Develop new referral sources and community partnerships Maintain communication with existing accounts to support continued referral growth Represent the facility at networking events, community events, and healthcare meetings Collaborate with admissions and clinical teams to support census growth goals Maintain knowledge of skilled nursing services, payer sources, Medicare, Medicaid, and insurance processes Track outreach activity and referral trends Qualifications: Previous healthcare marketing, outreach, admissions, or business development experience required Skilled nursing facility or long term care experience strongly preferred Knowledge of Medicare, Medicaid, managed care, and insurance processes required Strong communication and relationship-building skills Self-motivated with the ability to work independently and collaboratively as part of a team.
