Job Description
OverviewWe are seeking a compassionate and organized Care Coordinator to join our dedicated Licensed Home Care agency team.The ideal candidate will play a vital role in coordinating and scheduling patient care, ensuring staffing of each case, communicate with and coordinate caregivers ( HHA/PCA) and supporting individuals with diverse needs in home care setting.This position offers an opportunity to make a meaningful difference in the lives of home care patients and their families by providing exceptional support and care coordinating.The Care Coordinator will work closely with Nursing and caregiving professionals, patients, and families to ensure scheduling of caregivers, and care coordination needs are met, promote well-being and safety of the patient.Responsibilities Scheduling and staffing of the case Coordinate services across multiple providers, including nursing, home health, hospice, assisted living, and community care programs.Observe and document safety of the provided care.monitor caregiving assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, meal preparation, and mobility assistance using equipment like Hoyer lifts or resident lifts.Support compliance with HIPAA regulations and healthcare protocols.Educate patients and families about care plans, safety procedures, and health maintenance strategies.Orient caregivers on agency policies, appropriate reporting system, EVV.Qualifications Prior experience in working at Licensed Home Care Agency.Strong communication skills for working effectively with patients, families, healthcare providers, and team members.Knowledge of HIPAA regulations to ensure confidentiality of patient information.Compassionate attitude with a strong commitment to providing high-quality home care.This role offers an enriching environment for those passionate about caregiving and making a positive impact on individuals' lives through dedicated support and comprehensive care coordination.Job Type:Full-time Pay:$25.00 - $26.50 per hour Benefits:Health insurance Mileage reimbursement Paid orientation Paid time off Experience:Patient coordination, scheduling:1 year (Required) Language:English and Spanish (Required) Ability to Commute:Bay Shore, NY 11706 (Preferred) Work Location:In person.
