Job Description
Home Health Aide (HHA)/Caregiver You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Licensed HHAs/CNAs are encouraged to apply and we also welcome motivated candidates willing to become certified. This is a high-impact, patient-facing caregiving role. You'll be a trusted presence in the home helping clients stay safe, comfortable, and independent. Your work directly supports quality of life for patients and peace of mind for their families. Key Responsibilities Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan Prepare meals and assist with feeding as needed Support medication self-administration through reminders (no independent med administration) Monitor and report patient condition and changes to the supervising nurse Take and record basic vital signs when trained and assigned Maintain a clean, safe, and orderly client environment Assist with linens and light client laundry as appropriate Use safety equipment properly (rails, supports, stools, etc.) Document care provided according to company and regulatory standards Travel reliably to patient homes and assigned visits across your service area Complete required in-service training, including annual dementia education What Success Looks Like Patients feel safe, respected, and well cared for On-time arrival and dependable attendance for assigned visits Accurate, timely care documentation Positive feedback from patients and families Completion of required certifications and training on schedule Strong communication with supervising nurse and care team Must-Have Requirements Age 18 or older 1 full year of caregiving experience within the last 2 years, performed for a licensed agency under RN supervision. Willingness to complete competency and skills assessments Ability to follow written care instructions and document services Compassion, patience, and professionalism with vulnerable populations Ability to work independently in home settings Reliable transportation and ability to travel to client homes Ability to lift up to 25 pounds Willingness to complete background screening and onboarding requirements. Nice-To-Have Active HHA or CNA certification or willingness to obtain certification Home health or home care experience CPR and Adult First Aid certification Experience supporting seniors or medically complex clients Experience working with dementia or memory care patients Work Environment & Schedule One-on-one care in patient homes Flexible PRN/As Needed scheduling options Assignments may include homes with pets or smokers Travel within your preferred coverage area You may be a great fit if you: Are licensed or motivated to become licensed Enjoy helping others and building trust with clients Are calm, respectful, and safety-focused Can handle sensitive or challenging situations professionally Are dependable and punctual Communicate clearly with patients, families, and nurses Want meaningful, people-centered work If you're ready to make a daily difference in someone's life we'd love to meet you.
