Role Overview: We’re looking for a dedicated Case Manager to coordinate personal care services for clients in need. You’ll serve as the primary point of contact for families, ensuring they receive the right care at the right time. Your responsibility will be to build relationships with referral sources and community partners, and our mission is to connect families in need with our home health services, helping children and seniors receive the care they deserve. Key Responsibilities: Develop and maintain relationships with physicians, clinics, case managers, and community organizations. Identify and engage potential referral partners for pediatric and geriatric care. Educate partners on Etairos Health’s services and values. Track outreach efforts and maintain accurate records in CRM tools. Collaborate with internal teams to ensure smooth onboarding for new clients. What We’re Looking For: Background in social work, healthcare sales, or case management preferred. Excellent organizational and communication skills.
