Facility: BayCare HomeCare Location: Tampa, FL Status: Full Time Shift Hours: 8:00 AM - 5:00 PM / Monday - Fridays Weekend Work: 1 Weekend Rotation a month On Call: Yes The Home Health RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include: Oversee the activities of the multi-disciplinary team that provides care to the patient Monitor the provision of therapeutic services through written and verbal communication with all disciplines Ensure that all visits scheduled and provided are authorized and covered by physician orders Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits Inform the scheduler of any schedule changes and communicates caseload and patient care issue
