Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Manage complex wound care procedures such as dressing changes and catheter care while ensuring aseptic techniques are maintained. Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems like Epic or Cerner to document patient progress accurately. Provide education on medication management, nutrition, disease processes like Alzheimer's or dementia care, and self-care strategies to patients and their families. Monitor patients' responses to treatments such as pain management or respiratory therapies like ventilator support; adjust care plans accordingly. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for all patient interactions. Skills Proven experience with home health nursing, including long-term care, hospice, or post-acute settings. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in patient assessment, vital signs monitoring, medication administration, and IV insertion/injection techniques. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation and case management. Experience working with diverse populations including seniors with dementia or Alzheimer's disease; adept at providing compassionate memory and behavioral health support. Ability to perform complex procedures like catheterization, sterile processing, or spinal taps with precision and aseptic technique. Excellent communication skills for triage assessments and working effectively with multidisciplinary teams in urgent or emergency situations such as ICU or Level I Trauma Center environments.
