Home Health Nurse (Contracted) NEW GRADS WELCOME

PRN Assessment RN (Contract) — Colorado NEW GRADS WELCOME Overview Tree of Hope is hiring PRN Assessment Registered Nurses to perform home assessments across Colorado. This is a contracted position paying $125 per assessment. We offer flexible scheduling and will work around your availability. New grads are welcome to apply; mentorship available. Typical work split is ~80% field visits and ~20% office/home administrative time (varies seasonally). Position may be in-office or hybrid. Key Responsibilities Perform home health assessments (Start of Care, Recertifications, Discharges) Complete change-of-condition visits Conduct Pediatric Home Health Assessment Tool evaluations Provide nursing interventions as indicated Supervise and verify CNA skills Obtain physician orders and coordinate care with the care team Complete required documentation and administrative tasks remotely or in office Qualifications (Minimum) Active RN license (Colorado) Minimum 1 year clinical experience in home health services or similar setting perferred New grads welcome to apply; mentored/limited roles considered Current BLS certification Reliable transportation and ability to travel throughout assigned Colorado regions Strong assessment, communication, and documentation skills Preferred Experience supervising CNAs Pediatric home-health experience Familiarity with home health regulations and documentation systems Compensation & Work Arrangements $125 per assessment (contracted) PRN schedule — flexible; we will work around your availability In-office or hybrid options available How to Apply Send resume, current RN license, and brief availability to Intake@treeofhopecolorado.com .
