Home Health Nurse

Job Summary Join our dedicated healthcare team as a Home Health Nurse, where your expertise will make a meaningful difference in patients' lives by providing compassionate, comprehensive care in their home environments.This dynamic role offers the opportunity to deliver personalized nursing services, coordinate care plans, and promote health and wellness across diverse patient populations.Your clinical skills and empathetic approach will help individuals achieve optimal health outcomes while enjoying the flexibility and variety that home health care offers.Key Responsibilities:Patient Assessment and Care Planning:Conduct initial and ongoing assessments to evaluate patients' physical, emotional, and social needs.Develop and implement personalized care plans based on assessment findings and physician directives.Monitor patient progress and adapt care plans as needed.Patient and Family Education:Educate patients and families on medication adherence and self-care techniques.Provide guidance on maintaining a safe and healthy home environment.Assist caregivers with techniques to support the patient's ongoing care and comfort.Documentation and Reporting:Maintain accurate and thorough documentation of patient care, including assessments, interventions, and progress notes.Report any changes in the patient's condition to the supervising nurse or physician promptly.Submit required reports and documentation in a timely and accurate manner Qualifications:Bachelor's Degree Active NYS Registered Nurse License Homecare experience preferred, but not required Strong clinical and critical thinking skills Reliable, compassionate, and professional demeanor Experience and Knowledge of CMS/DOH Home Health Rules and Regulations Experience and Knowledge of Home Health Policies and Regulations Experience in the coordination of care, doctors' orders/485s Ability and willingness to travel in the five boroughs.This position description is a guide to the job's critical duties and essential functions, not an all-inclusive list of responsibilities and qualifications.Job Types:Full-time, Part-time Pay:$35.00 - $45.00 per hour Expected hours:40 per week Work Location:In person.
