Opportunities for Growth Expand your clinical expertise through ongoing education, mentorship, and professional development.Position Overview We are seeking a compassionate, motivated, and skilled Physical Therapist (PT) to join our expanding Home Health team.In this role, you will provide individualized rehabilitative care to patients in the comfort of their homes while helping them achieve functional goals, prevent decline, and improve overall quality of life.This is an ideal opportunity for clinicians who enjoy autonomy, meaningful patient interaction, and making a lasting difference in their community.What You'll Do Perform comprehensive patient evaluations and develop individualized Plans of Care Deliver high-quality physical therapy interventions based on physician orders and patient goals Help patients improve mobility, strength, balance, endurance, and safety Educate patients and caregivers on exercises, fall prevention, and safe mobility techniques Monitor patient progress and modify treatment plans as needed Collaborate closely with physicians, nurses, case managers, and interdisciplinary team members Maintain timely and accurate documentation in compliance with agency and state regulations Participate in case conferences and contribute to quality patient outcomes Supervise and mentor Physical Therapist Assistants (PTAs) when applicable Advocate for patient independence, dignity, and overall well-being Coverage Areas:We currently serve patients throughout Inland Empire Flexible territory assignments may be available based on your location.Benefits & Perks Full-Time Employees Medical Insurance Dental Insurance Vision Insurance Paid Time Off (PTO) Supportive team environment Continuing education opportunities Career advancement potential Per Diem Employees Paid Time Off Mileage Reimbursement Flexible scheduling What We're Looking For Required Qualifications Current California Physical Therapist License Valid Driver's License Reliable transportation with current auto insurance Ability to travel to patient homes within assigned territories Excellent communication and interpersonal skills Ability to safely assist and transfer patients as needed Preferred Qualifications Home health experience strongly preferred Experience in hospital or outpatient rehabilitation settings Bilingual skills are a plus Experience using EMR systems Required Credentials (All credentials must remain active and current) California PT License Driver's License & Auto Insurance TB Test or Chest X-Ray Physical Examination CPR Certification Make a Difference Where It Matters Most At Tender Home Health, your work goes beyond therapy visits - you help patients regain confidence, restore independence, and improve their everyday lives.If you're looking for a rewarding opportunity with flexibility, purpose, and a team that truly values you, apply today and become part of something meaningful.Pay:$80.00 - $110.00 per hour Application Question(s):Are you a licensed Physical Therapist (PT) in the State of California? Do you have a minimum of 6 months of Home Health experience as a PT? Are you willing to travel within a 20-30 mile radius to visit patients in their homes?
