Job Description
Job Type Part-time Description The Home Health Physical Therapist provides high-quality, patient-centered physical therapy services to individuals in their homes. You will be responsible for evaluating, diagnosing, and treating patients with various physical impairments, functional limitations, and mobility issues. In this role you will create personalized therapy plans, implement therapeutic interventions, and work closely with patients and their families to achieve optimal functional outcomes. Schedule: PRN; Part-time, Weekdays; Weekends Benefits: Mileage reimbursement Primary Responsibilities and Duties: Conduct thorough evaluations of patients' physical abilities, limitations, and needs to develop individualized therapy plans. Develop and implement effective treatment programs to improve patients' mobility, strength, flexibility, and overall functional performance. Utilize therapeutic exercises, manual therapy techniques, modalities, and assistive devices to address specific physical impairments and goals. Educate patients and their families on exercise techniques, home exercise programs and strategies to support recovery and enhance daily living activities. Monitor and document patient progress, responses to therapy, and any changes in condition providing detailed reports to the supervising physicians and other healthcare team members. Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated approach to patient care. Provide guidance on the use of assistive devices and home modifications to improve patient functional independence. Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards. Participate in the development of care plans and case c
