Home Health Physical Therapist

St. David's Medical Center seeks a Home Health Physical Therapist to provide high-quality, patient-centered rehabilitation services in patients’ homes across the Austin area. In this role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, and collaborate closely with physicians, nurses, and caregivers to promote optimal functional recovery and safety. You’ll educate patients and families, document care thoroughly, and contribute to a culture of excellence, compassion, and teamwork while managing an organized home-health caseload.
