Presbyterian SeniorCare NetworkHome Health is a Medicare-certified home health agency that provides in-home care and services when a physician prescribes for them. Managed by Redstone Presbyterian SeniorCare, our home health professionals make a difference in the lives of older adults by providing skilled nursing services; and physical, occupational, and speech therapy while our patients remain in a comfortable, at-home setting. The Social Worker assists and counsels patients and families with financial, social, and emotional concerns in a Home Health setting.In this role, the Social Workerwillaidpatients and their families with personal and environmental difficulties that couldinterfere with obtaining maximum benefits fromhome health care. The position will also be responsible for coordination of dischargeplans forresidents transitioning to home care and willprovide clinical supervision of all social work assistants providing home health care services at Presbyterian SeniorCare Home Health. Requirements For TheHome Health Social Worker Have a Master's degree in Social Work from an accredited college and a minimum of 1 year of Social Work in a health care setting; Be licensed as a Social Worker in the state of Pennsylvania or meet allthe education and experience requirements for licensure; Possess a valid driver's license and maintain reliable transportation that is insured in accordance with Pennsylvania requirements; Maintain current CPRcertification; Be able to effectively interact with residents, resident families, and team members; Have working knowledge of Internet Software, Spreadsheet Software, and Word Processing Software.
