Your core responsibilities include: First Point of Contact: Professionally manage all incoming calls and swiftly process referrals. Admission Coordination: Efficiently coordinate and schedule patient evaluations. FinancialRegulatory Management: Verify all Medicare, Medicaid, and private insurance benefits, including securing necessary authorizations. DocumentationData Integrity: Create, manage, and maintain accurate electronic medical records (EMR) and assemble all patient medical records and admission documents with precision. Seamless Communication: Ensure smooth coordination by communicating proactively with referral sources and community partners. Operational Support: Provide essential clerical and administrative support to maintain seamless organizational operations. Compliance: Uphold full compliance with all Medicare, State, Federal, and accreditation standards. What Success Looks Like In this role, you will know you are succeeding when: Referrals are processed quickly, accurately, and professionally. Patients and families experience a smooth, supportive intake process. Communication flows seamlessly between all teams and referral partners. All documentation is complete, compliant, and timely. You actively contribute to a positive, mission-driven team culture. Qualifications Required: High School Diploma or equivalent Proven experience in medical records or healthcare administration Exceptional organizational skills and meticulous attention to detail Strong verbal and interpersonal communication skills A professional, dependable, and team-oriented mindset Current CPR certification Preferred: Direct Hospice Intake or home health experience About Endeavor For over a decade, Endeavor Hospice has served the community with integrity and compassion, supporting patients and families through hospice care, providing in-home support services for seniors, and partnering with the DDD community through our HCBS division.
