Job Description
As a Hospice Nurse Practitioner with Gentiva, you will provide advanced, patient-centered care to individuals with life-limiting illnesses in their homes and community settings. Working within Gentiva’s healthcare and medical services team, you will conduct comprehensive assessments, develop and manage individualized care plans, and collaborate closely with physicians, nurses, social workers, chaplains, and other members of the interdisciplinary hospice team. You will focus on expert symptom management, pain control, and quality-of-life goals, ensuring care plans align with patients’ and families’ preferences and values. In this role, you will perform face-to-face clinical evaluations, obtain medical histories, perform physical examinations, order and interpret diagnostic tests as appropriate, and prescribe medications and treatments in accordance with state and federal regulations. You will provide clear education to patients and families about disease progression, hospice philosophy, treatment options, and advance care planning, supporting informed decision-making and emotional coping at every stage. Documentation will be completed accurately and promptly in the electronic medical record to support continuity of care and regulatory compliance. The Hospice Nurse Practitioner serves as a clinical resource and partner to the hospice care team, participating in interdisciplinary team meetings, reviewing ongoing care plans, and adjusting treatments based on patient response and goals of care. You will communicate regularly with referring and attending physicians, update them on clinical status, and coordinate transitions of care when needed. You will help ensure Gentiva’s commitment to compassionate, high-quality care is reflected in every interaction, while adhering to hospice regulations, agency policies, and accepted clinical standards.
