Key Responsibilities Respond to after-hours triage calls and assess patient needs Provide PRN in-home visits for urgent situations (symptom management, falls, death visits, etc.) Deliver compassionate end-of-life care to patients and support to families Perform clinical assessments and implement appropriate interventions independently Administer medications, including narcotics, safely and effectively Communicate with physicians, care teams, and medical examiners when required Document all care accurately using the EPIC EMR system Collaborate with the hospice team while working independently in the field Requirements Active Illinois RN License Minimum 2 years of Home Health or Hospice experience BLS (Basic Life Support) certification Valid driver's license, personal vehicle, and auto insurance Strong clinical assessment and critical thinking skills Experience handling end-of-life care and death visits Comfortable working independently in a home-based setting Ability to identify urgent symptoms and respond effectively Why Should You Apply?
