Provide an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided Counsel patient and family/caregivers
