Hospice Team Manager (Full Time) Monday to Friday, in our Fort Washington Office.The Hospice Team Manager oversees the IDT staff, coordinates, and manages IDT meeting.Provides clinical support and education to IDT members.Plays a vital role in the direct and indirect care of patients provided by clinical staff.Displays Leadership to the team and assists them in continually improving all aspects of hospice care services Responsibilities :Managing IDT meetings and ensuring compliance according to Federal and State Regulations.Providing educating to IDT members and serving as a mentor for ongoing improvement of care provided.Works directly with the Clinical Director and Program Director to ensure patient needs are being met.Establishing and overseeing caseloads of direct patient care members.Maintains charts according to regulations with oversight from Clinical Director.Most important is ensuring that patients and families receive care according to their needs to ensure supportive care through this journey.Qualifications:Must possess PA Nursing License, Hospice experience, a must!
