Job Description

Duties Administrative Support Answer and route incoming phone calls in a professional and compassionate manner Greet visitors, patients, and families and provide general assistance Manage office supplies, scanning, filing, and document organization Assist with data entry, scheduling support, and general clerical duties Clinical Team Support Support the interdisciplinary team with coordination of patient services Assist with scheduling visits and communicating updates to staff Prepare and organize patient charts, admission packets, and compliance documentation Ensure documents are complete, signed, and uploaded timely Coordination & Communication Serve as a communication liaison between patients, families, facilities, and staff Relay messages accurately and in a timely manner Support coordination of admissions, discharges, and care transitions Assist with tracking orders and ensuring follow-through Documentation & Compliance Maintain accurate and organized records in the electronic medical record (EMR) Assist in ensuring documentation meets hospice regulatory standards Protect patient confidentiality and comply with HIPAA guidelines Office Operations Support daily office workflow and assist leadership with administrative projects Help maintain a professional, organized, and welcoming office environment Assist with meeting preparation, including IDG meetings and staff meetings · Attend IDG meetings as appropriate · Provide community outreach and education · Assist with general orientation of staff and volunteers · Must complete a minimum of 6 hours of initial training in the first 60 days of employment as required by the Alzheimer’s Disease and Related Dementia’s Services Act. · Other duties as delegated by Supervisor. Experience Prior experience in an administrative or clerical role preferred; experience in healthcare or hospice settings is a plus. Strong organizational skills with the ability to multitask efficiently in a busy environment. Excellent communication skills—empathetic listening and clear verbal expression are essential. Proficiency with office software such as Microsoft Office Suite (Word, Excel, Outlook). Ability to maintain confidentiality and demonstrate professionalism at all times.
