Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patients’ vital signs and observe for any changes in health status, promptly reporting findings to healthcare professionals Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking to promote healthy living Administer medications accurately following care plans while maintaining strict adherence to HIPAA r Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to support overall well-being Support clients with specialized needs such as dementia, Alzheimer’s, autism, or developmental disabilities through dedicated memory care and behavioral strategies Document care activities thoroughly in compliance with healthcare standards and assist in updating individual care plans regularly Qualifications Proven experience in home health, assisted living, nursing homes, or group home settings with a focus on senior or disability care Knowledge of patient observation techniques, medication administration, vital signs monitoring, and emergency response procedures including CPR and first aid certification Familiarity with specialized care such as Alzheimer’s, dementia, hospice care, or working with individuals with developmental disabilities or autism spectrum disorder Ability to perform heavy lifting safely using equipment like Hoyer lifts when required Strong communication skills to collaborate effectively with clients’ families, healthcare teams, and social workers Valid certifications in CPR and first aid; additional training in HIPAA compliance preferred Flexibility to work various shifts including evenings or weekends as needed; experience working in home & community care environments is highly valued Embark on a fulfilling career where your compassion fuels positive change!
