Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients’ vital signs and observe for any changes in health or behavior Administer medication as prescribed, ensuring compliance with HIPAA regulations and care plans Prepare nutritious meals tailored to individual dietary needs and preferences Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges Maintain detailed documentation of patient care, observations, and medication administration in compliance with healthcare standards Support clients with memory care needs, including Alzheimer’s and dementia care, fostering a safe environment Perform light housekeeping tasks such as cleaning and laundry to ensure a tidy living space Collaborate with healthcare professionals to develop and follow personalized care plans Provide emotional support and companionship to promote mental well-being Follow all safety protocols including CPR and first aid procedures during emergencies Assist clients with community outings or social activities to enhance quality of life Experience Prior experience in home health, assisted living, nursing homes, or group home settings preferred Demonstrated ability to work with individuals with developmental disabilities, autism, or dementia-related conditions Knowledge of patient observation techniques and vital signs monitoring required Experience with medication administration and care planning essential Familiarity with HIPAA regulations ensuring confidentiality of patient information Hands-on experience using mobility aids such as Hoyer lifts or resident lifts advantageous Strong communication skills for working effectively with clients, families, and healthcare teams Ability to perform heavy lifting safely when assisting clients with mobility or transfers Background in social work or hospice care is a plus but not mandatory Join us to be part of a dedicated team committed to providing exceptional home care that fosters independence, safety, and comfort.
