Registered Nurse (Home Health) – Louisville Metro Assignment Length: 12 weeks Schedule: Monday–Friday, 8:00 AM–4:30 PM Weekly Hours Guaranteed: 32 On‑Call: One weekend per month Dress Code: Scrubs (any color) Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last Coverage Area: Louisville Metro Position Overview We are seeking an experienced Home Health Registered Nurse (RN) to deliver high‑quality, patient‑centered skilled nursing care throughout the Louisville Metro area. The RN will provide care in accordance with physician orders and individualized plans of care, ensuring patients receive exceptional support in the home setting. This role includes completing Start of Care (SOC) visits, routine assessments, and ongoing clinical evaluations. Key Responsibilities Deliver skilled nursing care aligned with clinical standards and physician directives Complete Start of Care (SOC) visits, routine assessments, and ongoing patient evaluations Provide patient and family education to support care goals and safety in the home Complete OASIS documentation accurately and on time Meet productivity expectations of 30 points per week Coordinate care with the interdisciplinary clinical team to ensure continuity Accurately document all patient interactions and clinical findings Required Qualifications Active RN license Minimum one year of home health experience Proficiency with Homecare Homebase (HCHB) Strong clinical assessment and documentation skills Preferred Skills (Highly Beneficial) OASIS certification Experience with CHF management Wound care expertise IV therapy skills By providing your phone number, you consent to: (1) receive automated text messages and calls from the Judge Group, Inc. and its affiliates (collectively "Judge") to such phone number regarding job opportunities, your job application, and for other related purposes.
