Job OverviewWe are seeking a compassionate and dedicated Home Care Aide to provide exceptional in-home support to individuals requiring assistance with daily activities and healthcare needs.The ideal candidate will possess a strong commitment to patient-centered care, demonstrating professionalism, empathy, and attention to detail.As a Home Care Aide, you will play a vital role in enhancing the quality of life for clients by delivering personalized care in their homes or community settings.This position offers an opportunity to make a meaningful difference while working in a dynamic and supportive environment.Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.Monitor and observe patients for changes in condition, documenting vital signs and health status accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.Administer medications as prescribed, ensuring proper medication management and adherence to HIPAA regulations for patient confidentiality.Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies.Support clients with medical procedures such as catheter care, wound dressing, and hospice-related needs under supervision or as trained.Operate medical equipment like Hoyer lifts safely to assist with patient transfers and mobility.Prepare nutritious meals, assist with feeding when necessary, and maintain cleanliness through light housekeeping tasks such as laundry and cleaning to ensure a safe living environment.Collaborate with healthcare professionals to update medical documentation and ensure continuity of care within home & community care settings.Maintain compliance with all safety protocols including CPR certification, medical terminology knowledge, and infection control standards.Engage in ongoing training related to long-term care, hospice care, working with individuals with disabilities, and other specialized caregiving areas.Skills Strong patient observation skills to detect changes in health status promptly.Experience working in assisted living facilities, nursing homes, or home health environments.Proficiency with EMR/EHR systems for accurate documentation of patient records and medical history.Knowledge of assisting with ADLs, medication administration, catheter care, and vital signs measurement.Familiarity with dementia care techniques, Alzheimer's care strategies, and memory care approaches.Ability to operate medical equipment such as Hoyer lifts safely and effectively.Excellent communication skills for engaging with clients, families, and healthcare teams professionally and compassionately.Understanding of medical terminology relevant to home health caregiving roles.Join our team as a Home Care Aide to deliver compassionate support that empowers individuals to live safely and comfortably in their own homes while advancing your career in healthcare support services!
