Licensed Care Manager

A company is looking for a Care Manager. Key Responsibilities Develop trusting relationships with members to support health behavior changes using motivational interviewing techniques Assist in coordinating care and provide educational resources and recommendations for follow-up care Create individualized health plans and serve as a clinical resource for Care Management team members Required Qualifications, Training, and Education Bachelor's Degree in Occupational Therapy (OTR) or Associate's Degree (OTA or PTA) Minimum 2-4 years of clinical experience with patient assessments and care plan creation Experience supervising OTAs and working with the Medicare population Familiarity with Motivational Interviewing techniques and behavioral change theory Experience working with geriatric or frail populations
