ABOUT US My Brother's Keeper is a substance use disorder treatment facility providing medically supervised detoxification and residential inpatient care to adults in Eastern Kentucky. Operated by Phoenix Rehabilitation Center, LLC, we deliver evidence-based, trauma-informed treatment with clinical excellence and genuine compassion. Our team includes a board-certified addiction medicine physician, licensed therapists, social workers, and 24-hour nursing staff. We treat alcohol, opioid, methamphetamine, and polysubstance use disorders, with integrated dual diagnosis capability for co-occurring mental health conditions. We are located in Mount Vernon, Rockcastle County — at the center of a region significantly impacted by the addiction crisis — and we take that responsibility seriously. POSITION SUMMARY We are seeking a Licensed Clinical Social Worker (LCSW) to join our clinical team. You will provide individual psychotherapy, facilitate therapeutic groups, complete biopsychosocial assessments, and contribute to individualized treatment planning for patients in our medical detox and residential inpatient programs. You will work alongside a board-certified addiction medicine physician, licensed therapists, and dedicated case managers. KEY RESPONSIBILITIES Clinical Assessment & Treatment Planning Conduct comprehensive biopsychosocial assessments on newly admitted patients Develop individualized treatment plans in collaboration with the multidisciplinary clinical team Identify and document co-occurring mental health diagnoses and integrate into the treatment plan Complete required clinical documentation accurately and within established timeframes Individual Therapy Provide scheduled individual psychotherapy sessions to an assigned caseload Utilize evidence-based modalities including CBT, Motivational Interviewing, Trauma-Focused CBT, and DBT skills as clinically indicated Monitor and document patient progress toward treatment goals Group Therapy & Psychoeducation Facilitate structured therapeutic groups including relapse prevention, coping skills, emotional regulation, grief and loss, and trauma recovery Develop and deliver psychoeducational programming aligned with patient population needs Dual Diagnosis Treatment Assess and treat co-occurring mental health conditions — depression, anxiety, PTSD, bipolar disorder — as part of an integrated approach Coordinate with prescribing staff on psychiatric medication management Discharge Planning & Aftercare Initiate discharge planning at admission and update throughout treatment Connect patients with outpatient treatment, sober living, community mental health, and peer support services prior to discharge Complete discharge summaries and transition documentation per regulatory standards Documentation & Compliance Maintain records in compliance with HIPAA, 42 CFR Part 2, and Kentucky licensing standards Participate in treatment team meetings, clinical supervision, and quality improvement Support Joint Commission accreditation readiness REQUIRED QUALIFICATIONS Active, unrestricted LCSW license issued by the Kentucky Board of Social Work Master of Social Work (MSW) from a CSWE-accredited program Minimum 2 years post-licensure clinical experience in behavioral health or substance use disorder treatment Demonstrated competency in individual and group psychotherapy Proficiency in CBT, Motivational Interviewing, DBT, and trauma-informed care Strong clinical documentation skills Knowledge of HIPAA and 42 CFR Part 2 confidentiality regulations PREFERRED QUALIFICATIONS Experience in a residential inpatient or medical detox setting Dual diagnosis treatment experience Specialty certification (CADC, CCTP, or trauma certification) Familiarity with ASAM criteria and level-of-care determination Knowledge of Eastern Kentucky community resources Pay: $55,000.00 - $75,000.00 per year Work Location: In person
