Job Description Craft Customized Treatment Plans: Assess patients' unique needs and develop tailored strategies to assist them in reaching their individual functional goals. Collaborate with Multidisciplinary Teams: Work alongside OTs, PTs, STs, medical professionals, and nursing staff to provide holistic, comprehensive care. Educate Patients and Families: Share knowledge regarding therapy methods with patients and their families to foster continued recovery at home. Track Progress and Make Adjustments: Monitor patient progress, document clinical outcomes, and modify treatment plans as necessary to keep patients on the right path.
