LPN LVN - Home Health Schedule- Full Time/Days The LPN provides quality care to patients in the nursing center and their homes under the direction of a Nurse Case Manager or Clinical Supervisor following the established nursing process and procedures, and in accordance with the Nurse Practice Act. Performs all duties in compliance with all applicable State and Federal regulations and in compliance with all regulatory bodies. LPN's working in Home Health require 25% to 35% of time on daily travel. Education to meet certification, license or registration requirement. ~ Oregon Licensed Practical Nurse upon hire or, ~ Caregivers will be expected to provide proof of driver license and auto insurance upon request. ~ License in states wherever care is delivered upon hire. ~1 year Experience in Acute Care, Home Health, public health nursing, or patient care experience in a skilled nursing facility or medical clinic.
