Interim HealthCare is seeking a compassionate Licensed Practical Nurse to provide skilled nursing care to patients in their homes within the Healthcare & Medical Services field. As an LPN with Interim HealthCare, you will deliver patient-centered care tailored to individual needs, supporting patients with chronic conditions, post-acute recovery, and ongoing health management. Working closely with RNs, physicians, therapists, and caregivers, you will perform comprehensive patient assessments within your scope of practice, monitor vital signs, administer medications and treatments, dress wounds, and assist with activities of daily living while promoting comfort, safety, and dignity. You will implement and follow established care plans, update clinical notes accurately and promptly using electronic medical records, and communicate significant changes in patient status to the supervising RN and healthcare team. You will educate patients and families on disease processes, medications, and home care techniques, reinforcing instructions to support independence and prevent avoidable hospitalizations. In this role, you will coordinate visits, manage your schedule efficiently, and travel between patient homes, demonstrating professionalism and respect in every interaction.
