Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate care and support primary care practice in Independent living. Your skills will help ensure safe and effective care for our geriatric population. This position offers an exciting opportunity to apply your clinical knowledge. This position is primarily based at a single facility, with limited travel (within a 10‑mile radius) and a strong focus on coordinating care with physicians and NPs. Duties Deliver comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses to treatment plans. Maintaining meticulous medical documentation in electronic health record (EHR) systems. Support triage activities by prioritizing patient needs and communicating with providers. Conduct comprehensive post‑discharge reviews, including reconciliation of hospital or rehab discharge paperwork and medication lists, to ensure safe transitions and reduce re‑hospitalization risk for residents Educate patients and families on health coaching topics such as dementia care, medication management, nutrition, and chronic disease control to empower self-care. Skills Demonstrates knowledge of practical nursing theory, modalities, treatments, and skills A high level of multitasking, organization, and proactive communication skills are necessary Problem-solving abilities Supports the prescribing physician and/or nurse practitioners as needed Positive interpersonal communication skills Must possess sound judgment and critical thinking skills Team and customer service oriented Must be able to manage multiple details, tasks, and decision-making abilities under busy and stressful working conditions Basic computer skills required including but not limited to MS Office; additional computer knowledge [Preferred] Venipuncture skills [Preferred] Must be proficient in the English language; additional languages [Preferred] Background check meets organizational guidelines Requires individuals to be able to make emergency and mature independent judgments and decisions Must have excellent communication skills, be flexible, and able to interact positively with associates Knowledge with a Point of Care software system for patient care documentation [Preferred] Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) and HIPAA compliance training. Proficiency with EMR/EHR systems such as Tebra and PointClickCare [Preferred]. Strong knowledge of anatomy, physiology, pharmacology, and medical terminology essential for patient assessment and diagnostic evaluation. Excellent communication skills with the ability to work with physicians and NPs while demonstrating compassion for patients with complex needs.
