Job Title: Licensed Practical Nurse (LPN) – Long Term Care Location: Grand Junction, CO Shift: Variable Nights Schedule: 36 Hours/Week (12-hour shifts, 3 days/week) Job Summary A dedicated and compassionate Licensed Practical Nurse (LPN) is needed to support a skilled nursing and long-term care environment. This role involves delivering high-quality patient care, administering medications, monitoring resident health, and assisting with individualized care plans. The ideal candidate is patient-focused, dependable, and able to work effectively in a team-oriented setting. Key Responsibilities Provide direct patient care in a long-term care setting Administer medications and treatments as prescribed Monitor and document residents’ health status and vital signs Assist in developing and implementing individualized care plans Collaborate with RNs, physicians, and healthcare staff Ensure accurate and timely clinical documentation Support residents’ comfort, safety, and overall well-being Required Qualifications Active Colorado LPN License Ability to work night shifts and flexible schedules Strong communication and patient care skills Preferred Qualifications Experience in Long-Term Care (LTC) or Skilled Nursing facilities For more details reach at vgill@navitassols.com or Call / Text at 516-862-1203 .
