Licensed Practical Nurse (LPN) – Home Care

Job Overview We are seeking a proactive Licensed Practical Nurse (LPN) to provide in-home care for an alert elderly patient. The ideal candidate will demonstrate initiative, excellent communication skills, and a compassionate approach to supporting the patient’s health and independence. Duties Remind the patient when medications are due and explain the purpose of each medication. Assist the patient with home exercises as prescribed by the physical therapist (PT), offering encouragement and guidance. Monitor and document the patient’s health status and report any changes to the healthcare team. Support daily living activities as needed, ensuring patient safety and comfort. Maintain accurate records of care provided and medications administered. Communicate effectively with the patient, family members, and other healthcare providers. Experience Current LPN license in the state of Florida. Minimum of one year experience in home care or geriatric nursing preferred. Knowledge of medication administration and patient education. Ability to assist with physical therapy exercises as directed by PT. Strong organizational and time management skills. CPR certification required.
