PRN LPN Field Supervisor – Home Care MCO Homecare Services LLC Ellenwood, GA 30294 Service Areas: Griffin, Hiram, Covington, Decatur, Conyers, and surrounding Georgia communities Job Type: PRN / Part-Time Work Location: On the road / In person at client homes Job Description MCO Homecare Services LLC is a growing home care agency serving clients throughout Georgia. We are currently seeking a dependable, professional, organized, and detail-oriented PRN Licensed Practical Nurse — LPN Field Supervisor to assist with home supervisory visits, caregiver oversight, caregiver evaluations, skills testing, documentation reviews, and other nursing-related duties in a home care setting. This position is ideal for an LPN who is compassionate, reliable, organized, and comfortable working independently in client homes while also supporting caregivers and helping ensure quality care is being provided. The LPN Field Supervisor will help ensure caregivers are following the client’s care plan, documenting properly, and providing safe, appropriate, and compassionate care. The ideal candidate must be able to complete and submit all required documentation accurately and in a timely manner, preferably within 24–48 hours after the completion of each visit or assigned task . Our work environment includes Flexible scheduling Growth opportunities Supportive administrative team Client-centered care approach Professional and rewarding home care setting Responsibilities The LPN Field Supervisor may be responsible for the following duties: Complete home supervisory visits as assigned Monitor client condition and report concerns appropriately Review caregiver care logs, notes, and service documentation Complete annual caregiver performance evaluations Complete caregiver annual skills competency reviews Assist with caregiver education, coaching, and support within the LPN scope of practice Ensure caregivers are following the client’s care plan Identify and report client safety concerns, changes in condition, or service issues Communicate with the RN, Administrator, care team, clients, families, and caregivers as appropriate Assist with chart reviews, documentation audits, and quality assurance reviews Review and update client care records as needed Document all visits, evaluations, skills tests, and reviews accurately and timely Submit all required documentation within 24–48 hours after completing visits or assigned tasks Maintain HIPAA compliance and client confidentiality Follow agency policies, procedures, and applicable state regulations Perform other duties as assigned within the LPN scope of practice and home care setting Qualifications Current and active Georgia LPN license required At least 1 year of nursing experience preferred Home care, home health, long-term care, or geriatric care experience preferred Experience completing supervisory visits preferred Experience with caregiver training, skills checks, or competency evaluations preferred Strong documentation and communication skills required Must be dependable, professional, and organized Must be able to complete and submit documentation accurately within 24–48 hours Must be comfortable traveling to client homes Must be able to work independently and manage time effectively Must have reliable transportation Valid driver’s license preferred CPR/First Aid certification preferred Updated negative TB test required; if TB test is positive, a cleared chest X-ray is required Bilingual is a plus, but not required Preferred Experience Home care or home health experience Experience working with seniors or individuals with disabilities Experience reviewing caregiver documentation and care logs Experience completing caregiver evaluations or skills testing Experience using electronic documentation systems Knowledge of care plans, caregiver oversight, and client safety monitoring Schedule PRN / as needed Part-time hours available Flexible schedule based on client and agency needs Daytime availability preferred Travel to client homes required Benefits Fle
