Provide in-home evaluations and treatments to improve communication, cognition, and swallowing safety. Develop personalized care plans and educate patients and caregivers. Collaborate with healthcare team members and document progress accurately. Work independently in a flexible schedule with support from clinical mentors. Qualifications: Active SLP license, valid driver’s license, reliable vehicle, and EMR experience preferred.
