Location: 1601 Petersen Ave, San Jose, CA 95129 Website: https://westwoodpostacute.com/ We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing facility team. RESPONSIBILITIES Administering medications and monitoring patient reactions Assisting with daily living activities such as bathing, grooming, and feeding Maintaining accurate patient records and documentation Collaborating with RNs and other healthcare team members to implement care plans Conducting routine physical examinations and health assessments Educating patients and families about health conditions and care plans Ensuring compliance with safety and health regulations and protocols QUALIFICATIONS: Fresh or newly licensed LVNs are welcome Experienced LVN is preferred Experience in a skilled nursing facility/long-term care/senior care is a plus Valid and active Licensed Vocational Nurse (LVN) license in the state of California.
