Description The Local Intellectual and Developmental Disability Authority (LIDDA) Division of StarCare is in search of qualified, compassionate Service Coordinators seeking to make a difference in the lives of people. Responsibilities of LIDDA Service Coordinators include: Provide assessment, service planning, coordination, and monitoring of services for people on assigned caseload. Conduct activities through face-to-face and telephone contacts with individuals, providers, family members/legal guardians. Assist people on an assigned caseload to establish and/or maintain eligibility for services. Promote Person Centered Thinking and self-determination by educating individuals about rights, options, and choice. Complete and submit all required program documentation in accordance within the Local IDD Authority (LIDDA) Department's designated timeframes. Ensure all activities support and promote StarCare's culture of hope and health. Schedule: Full-time, non-exempt positions scheduled Monday - Friday 8am - 5pm with flexibility to meet the needs of the program.
