LISCW Social Worker

Bethany Health Care is looking for a Social Worker for a Skilled Nursing Facility. 24 hours per week Salary up to $40.00 per hour, based on experience and credentials JOB SUMMARY Assists in meeting the psychosocial needs of residents in a long-term care facility. Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life. This role will QUALIFICTIONS MA LICSW required. A minimum of 2-3 years' experience in a long-term care setting. Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques. Excellent communication, interpersonal, and computer skills required. Knowledge of electronic medical records, MDS, and Care Plans. We are looking for a candidate with a positive, compassionate, empathetic attitude, and an affinity for working with the elderly.
