Nurse Practitioner | Surgery - General/Other Location: Santa Rosa, CA Employer: Alumni Healthcare Staffing Pay: Competitive weekly pay (inquire for details) Contract Duration: 13 Weeks Start Date: ASAP About the Position Specialty State CA Location 1165 Montgomery Dr., Santa Rosa, CA 95405 Requirements LOCUM- General Surgery APP Please note trauma is a large part of this position, please ensure your providers are comfortable WE CAN ONLY LOOK AT PROVIDERS WITH CLEAN FILES, IF YOU PRESENT A PROVIDER WITHOUT A CLEAN FILE WE WILL HAVE TO DECLINE JOB DETAILS: _______________________________________________________________ We are looking for 1-2 providers to help with urgent coverage in Santa Rosa from June 1st to August 31st. Provider must have at least 2 weeks on/2 weeks off availability or more. Shifts are going to be 10 hours (8am-6pm). Must have a PA with general surgery experience. PA-C NPs can be consider but must have ACNP credential and be nationally certified. Please ensure bids reflect the below: - Guaranteed Hours: 8 per day - OT to apply after 8 hours per day REQUIREMENTS: ______________________________________________________________ ACTIVE CA license and CA DEA Board Certified OR experience BLS and ACLS certifications REQUIREMENTS: ___________________________________________________ Active CA License and CA DEA Board Certified or Board Eligible provider within 3-5 years of residency BLS and ACLS certifications TRAVEL REQUIREMENTS: _____________________________________________________ Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement): NOT reimbursed: o Mileage from provider’s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return. WILL be reimbursed: o Airport parking at provider’s departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips. Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.
