LA License Flu or Other Immunizations Required? Waiver? Required Items needed to request application: Active State Medical License, CDS, DEA, CV and COI Items needed for Credentialing: Active State Medical License, CDS, & DEA (with LA address), ACLS and ATLS certified (Unless ABEM-certified & actively participating in ABEM's continuing certification process).
