Doctor of Medicine | Family Practice Location: Fountain Valley, CA Employer: Elite365 Pay: Competitive weekly pay (inquire for details) Start Date: ASAP About the Position ASSIGNMENT DETAILS: Location: Ferndale, CA Start Date: ASAP Schedule: Monday- Friday 8a-5p Avg Patients/day: 18-20 Ages: Adults Cases: Bread and butter Primary Care EMR: EPIC Credentialing: 30-60 days QUALIFICATIONS AND REQUIREMENTS: Board-Certified Family Medicine MD/DO Minimum 2 years' exp in Primary Care Active CA License Active CA DEA Requirements • Duration of contract: 13+ Weeks, potential for locums to perm Practice Setting: Primary Care Coverage & Call Reqs: None Charting Software: Epic Ave. Daily Census: 18-20 Patient Age Group: Adults Procedures (if applicable): Great if you can do common outpatient IM procedures (biopsies, sutures, etc.) Equipment: Normal equipment found in a primary care outpatient office. Board Certifications: Preferred but not required License Certifications: Must have unrestricted CA medical license, DEA certificate, BLS or ACLS State License: CA Required CSR: Must have a DEA DEA: Must have a DEA Years of Exp.
