Job Description
Job OverviewWe are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Home Health care to join our dynamic team.The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a commitment to providing high-quality, personalized care in patients' homes.This role offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing health support, emphasizing independence and comfort within their community setting.The LPN will be responsible for delivering comprehensive nursing services, coordinating care plans, and ensuring adherence to all regulatory standards.Duties Perform detailed patient assessments, including vital signs, physical examinations, and medication reconciliation, utilizing EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems.Administer medications via injections, IV infusions, and other routes while adhering to HIPAA regulations and aseptic techniques.Provide wound care, catheterization, tube feeding management, and ventilator support as required for patients with complex needs.Conduct patient education on disease management, medication administration, and health coaching tailored to individual conditions such as Alzheimer's disease, dementia, or developmental disabilities.Collaborate with interdisciplinary teams to develop and implement personalized care plans that address acute pain management, infection control, and chronic disease management.Assist with specimen collection and processing, perform phlebotomy and venipuncture procedures accurately.Ensure compliance with safety protocols including basic life support (BLS), infection control practices, and documentation standards consistent with NCQA (National Committee for Quality Assurance) standards.Coordinate discharge planning and case management activities to facilitate smooth transitions between hospital or outpatient settings to home care environments.Experience Valid Pennsylvania Practical Nursing (LPN) license Proven experience working in diverse settings such as home health, nursing homes, assisted living facilities, or hospitals with exposure to critical care or emergency medicine environments.Background in managing patients with complex conditions such as infectious diseases, acute pain episodes, or post-operative recovery.Ability to travel to patient homes in the surrounding Warren county area.Pay:$23.30 - $25.16 per hour Benefits:401(k) 401(k) matching Dental insurance Employee assistance program Flexible spending account Health insurance Life insurance Paid time off Professional development assistance Referral program Retirement plan Vision insurance Work Location:In person.
