Interim HealthCare is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality home health services to patients in their homes. As an LPN with Interim HealthCare, you will deliver patient-centered nursing care that supports each individual’s unique needs, collaborating closely with RNs, therapists, and other members of the healthcare team. You will perform skilled nursing tasks such as medication administration, wound care, vital sign monitoring, chronic disease management, and treatment plan implementation under the direction of a Registered Nurse or physician. In this role, you will complete accurate clinical documentation, follow established care plans, and communicate changes in patient condition promptly to ensure safe and effective care. You will educate patients and families on disease processes, medications, and home care techniques, promoting independence and improving quality of life. Working in a community-based home health environment, you will travel to patient residences, manage your daily schedule, and adapt care to diverse home settings while upholding infection control standards and regulatory requirements.
