The ideal candidate for this position will have at least one year of experience in long-term care or home health, possess a friendly and professional demeanor and enjoy helping people. This is a rewarding position with many rewards such as a comprehensive benefits package. Responsibilities Maintain a professional relationship with patients and their families. Interpret residents' condition and needs in relation to the home environment. Make notes of residents' deaths, changes in resident status, appointments, and other pertinent information. Attend meetings and write reports for social workers and nurses. Communicate with family members about patient status.
