LPN / RN - Hourly Home Health Care

OverviewWe are seeking Licensed Nurses to provide high-quality, compassionate home health care services.This hourly position requires nurses experienced with spinal cord injuries and can work cooperatively with a home health aide (HHA).Ideal candidates will possess extensive clinical experience, strong assessment skills, and a commitment to improving patient outcomes while encouraging a positive, engaging team environment for the patient's emotional and psychosocial well-being.Candidates must be comfortable working independently in a home environment and collaborating often with a multidisciplinary team.As a Florida Home Health Care Provider, candidates are required to have a LEVEL II background screening via the Clearinghouse portal.For more information, please visit https://info.flclearinghouse.com.Duties Administer medications and manage treatments as prescribed, including but not limited to, injections, catheter care, wound care, dressing changes, colostomy care and IV infusions following sterile techniques and safety protocols.Follow individualized care plans in coordination with physicians, case managers, therapists and any others on the interdisciplinary team.Manage daily schedule and accompany patient scheduled HHA to personal appointments and/or social activities.Promote a positive/supportive environment to enhance the patient's emotional well-being.Document and communicate often/accurately patient interactions, matters, concerns and the overall care provided.Encourage participation with home exercise programs directed by licensed therapists.Assist home health aides as needed with transfers, feeding, light housekeeping and daily living activities.Educate patient on disease processes, medication management, nutrition, mobility aids like Hoyer lifts, and safety measures.Respond promptly to urgent needs such as autonomic dysreflexia, pain management or infectious disease control in the home setting.Ensure infection control practices are followed diligently during all patient interactions.Utilize Electronic Medical Record (Wellsky) to thoroughly document care consistent to orders and plan of care.Experience Clinical experience in diverse settings such as medical-surgical, Emergency room, or post-acute care environments.Prior experience with spinal cord injuries and currently IV certified.Proficiency in the importance of medical documentation and terminology.Knowledge of physiology, anatomy and pharmacology relevant to home health nursing.Job Types:Part-time, Per diem Pay:$28.00 - $30.00 per hour Benefits:401(k) Dental insurance Health insurance Paid time off Vision insurance Application Question(s):Are you willing to transport patient in his personal Sprinter Van for clinical appointments and/or social events?
